BECOME A MEMBER
Becoming a member is simple and easy.
STEP ONE: Print
and fill out the forms below. You can also pick these blank forms up
at the center, if you don't want to print them.

Waiver and Membership Contract
STEP TWO: Once the forms
are completed, either mail them, email them to us, or you can drop
them off in the drop slot outside our facility's door.
STEP THREE: Once we receive
your completed forms, we will enter your information, and send you your
keycard. We will also setup your membership fee payment with your bank
account. The membership fees will automatically renew on the first of
every month unless Pure Life Fitness Center is notified in writing.
Cancel Membership:
If a member wishes to
cancel a membership, they must fill out the Member
Cancellation Form and return their member's
keycard to to the facility through the drop
slot. Cancellation will take effect on the last day of the month.
